Charter Constant Disconnect

Hello, I have a charter combination modem (phone + internet). Recently, I have been noticing the internet AND phone go offline/no dial tone for a period of ~ 5 min and then come back online. Clearly this is not an issue with my router as the phone also goes offline. It is an ARRIS modem and I have provided some diagnostic info: Downstream Interface Name Freq Power SNR Modulation Downstream 1 655.750 MHz -4.30 dBmV 37.50 dB QAM256 Downstream 2 637.750 MHz -5.30 dBmV 37.20 dB QAM256 Downstream 3 643.750 MHz -5.10 dBmV 37.20 dB QAM256 Downstream 4 649.750 MHz -4.80 dBmV 37.20 dB QAM256 Upstream Interface Name Frequency Power Channel type Symbol Rate Modulation Upstream 1 29.00 MHz 48.00 dBmV ATDMA 5120 kSym/s QAM64 Does anything here indicate there is a problem with the ISP connection? Or do I simply need a new modem? Thanks for your help. Very much appreciated.
